Parental consent for immunisation
In Victoria, a consent card is provided by councils to schools. Students are required to take the cards home and have them completed and signed by their parent or guardian. Completed consent cards must be returned to the school before the student can receive immunisation. Consent cards contain information about the vaccine, the disease protected against and possible side effects.
In 2017, Year 7 students will be offered a 3 dose course of Human Papillomavirus, a chickenpox vaccine and a Boostrix vaccine (Diptheria, Tetanus, Whooping Cough).
These will be administered on:
• Friday 3rd March
• Friday 5th May
• Friday 8th September
Consent cards have been distributed in your child’s Learning To Learn class.
Please return to the front office, the tear-off consent card (Part A) with either ‘yes' or ‘no' answers before Monday 20th February.
